Meeting Room Request Form
Please read the Meeting Room Policy prior to requesting the use of the room.

All applicants must download, print, and sign the Hold Harmless Agreement prior to the meeting date. Applicants seeking a waiver of the insurance certificate requirement must obtain that from the Library Director prior to the meeting date.  Download Hold Harmless Agreement (.pdf) (policy change in effect March 14, 2016)

Non-profit organizations may download the paper form or submit the information in the form below.

Profit making entities should download and submit this special form.

Please make arrangements well in advance (days, not minutes!) if you wish to use the Library projection equipment. Only a few staff know how to set up and troubleshoot connections.
